Regular readers of the MJA might recall that, in the past, our final issue of the year once had a Christmas theme. As many Australians begin to wind down in December and look ahead to holidays and celebrations, the Journal traditionally filled the issue with articles replete with festive cheer, whimsy and humour. Not so for 2023. In choosing to retire the annual Christmas issue, we are recognising that so many of the things that the country cherishes and looks forward to over the summer holiday period — shared experiences with loved ones and our communities, rest and relaxation, enjoyment of our stunning and rich natural environment — are fundamentally intertwined with people's health and wellbeing. As editors of a medical journal committed to advancement of health, better, we thought, to spend our precious, limited pages showcasing content that strives to achieve this goal. This issue of the MJA is dedicated to planetary health. The way that humans interact with our natural world is arguably the most pressing health challenge of our time. Six articles in this issue address the myriad ways that human health and wellbeing critically depend on the earth's natural systems, the interlinked crises of climate change, biodiversity loss, and environmental degradation, and the responsibility that we all share for protecting and promoting a healthy and sustainable world. Since 2017, the MJA has partnered with Australian researchers and the Lancet Countdown to publish annual reports on the links between public health and climate change in Australia. In their editorial, Beggs and Zhang (doi: 10.5694/mja2.52152) draw out key findings for Australia from the latest global Lancet Countdown report and discuss the expansion of the Countdown's global data monitoring and analysis work to encompass regional centres in Asia, South America, Europe, and Small Island Developing States. In recognition of the importance of regional relationships, the next report of the MJA–Lancet Countdown, planned for early 2024, will broaden in scope to include a focus on Oceania. Similarly, two editorials in this issue highlight how global cooperation and advocacy efforts are crucial for addressing planetary health priorities. The Australian Global Health Alliance (doi: 10.5694/mja2.52166) argues “Australia is in a unique position to leverage its influence and align efforts in global health with the opportunities that responding to the climate change crisis provides”. The Alliance makes seven recommendations for actions the Australian global health community should implement to support a more sustainable and resilient future. And an article published in over 200 medical journals worldwide (doi: 10.5694/mja2.52148) calls for political leaders to recognise that the escalating climate and nature crises constitute “a global health emergency” and for “WHO to make this declaration before or at the Seventy‐seventh World Health Assembly in May 2024”. The important role of the health sector in attending to these challenges is highlighted by Pendrey and colleagues in a perspective (doi: 10.5694/mja2.52158) discussing new standards in Australian medical education that recognise planetary health, the health impacts of climate change, and the need for environmentally sustainable health practice, while a letter from Dunne and colleagues (doi: 10.5694/mja2.52159) explains why the decarbonisation of Australia's hospital system is both essential and achievable. Finally, research by Varghese and colleagues (doi: 10.5694/mja2.52171) estimates that collectively Australians lost 968 years of full health (disability‐adjusted life years) to heat‐related occupational injuries between 2014 and 2019. The highest burden of these injuries occurred in New South Wales and Queensland, and the authors warn that “without adaptive measures and industry‐based policies, the heat‐attributable occupational injury burden will increase as climate change advances”. At the end of the year, we also recognise all the efforts of the medical community in bringing together the Journal. The MJA aspires to be the journal of the Australian medical community, for the medical community, and that's reflected in the enormous number of people who contribute to it in one way or another. In the 12 months to the end of October 2023, we received 1391 new manuscripts (with 7705 authors, an average of five per paper), 588 of which were research and the rest a diverse mix of clinical cases, guidelines, reviews and opinions (Box). One of the joys of being a journal editor is that you get to read a wide diversity of articles; the saddest thing is that we are not able to publish or even peer review all that we receive. We accepted 20% overall of what we received, but just 7% of the research articles. Throughout 2023, we have refined internal processes to speed up and ensure consistency in our handling of articles, and we welcome feedback on these processes. We have updated our instructions for authors to make them clearer and easier to navigate, and have written new guidelines for reviewers (https://www.mja.com.au/journal/instructions‐authors‐reviewers). If you are writing or reviewing for us, please read these! We are very grateful to the Editorial Advisory Group members (https://www.mja.com.au/journal/staff/mja‐editorial‐advisory‐committee), 42 of them currently, and the 992 reviewers (pp 508–510) who gave their time and expertise to the medical community by advising and reviewing for us in the past 12 months, many on multiple occasions. 2024 will undoubtedly be a challenging year globally. At the time of writing, there are major conflicts ongoing in Ukraine and the Middle East, with many more regional ones. We have just lived through the hottest year on record in our planet's history. In Australia in 2023, the result of the referendum on the Indigenous Voice to Parliament, which the MJA supported (https://www.mja.com.au/journal/2023/219/6/mja‐supports‐aboriginal‐and‐torres‐strait‐islander‐voice‐parliament), was profoundly disappointing for many. We recognise that as a medical journal we have a central role in advocating for health — in its many meanings — across our society, and we look forward to continuing that work in 2024 and beyond.
