Assessment teams and procedures

"Shared care" programs for people with psychiatric disorders who live in the community are now common.
 Typically, a general practitioner manages the long term treatment of the patient's illnesses (mental and physical), with the assistance of case managers (such as community psychiatric nurses, working in community mental health teams) and advice from a psychiatrist.
 Shared care in the community has replaced institutional care for many people with psychiatric disorders.
 Home visits are a key aspect of patient care in these shared care arrangements.
For effective assessment and follow-up, it is important to work with other services and mental health professionals, such as community mental health care teams.
