1: Questions added to the Health Omnibus Survey to determine the use of asthma management plans

  1. Was your asthma confirmed by a doctor?
  2. Do you still have asthma?
  3. Do you have an asthma management plan?
    If yes, is this management plan written down for you completely or in part or did you receive verbal instructions?
  4. Have you been told by a doctor whether your asthma is mild, moderate, or severe?
    If yes, is it mild, moderate or severe?
  5. Have you ever discussed "trigger factors" with your local doctor who mainly looks after your asthma?
  6. Do you have an asthma action plan?
  7. Do you have a respiratory specialist or general practitioner or practice of GPs that you usually see for your asthma? How many times do you see this specialist or general practitioner a year?
