Was your asthma confirmed by a doctor?
- Do you still have asthma?
- Do you have an asthma management plan?
If yes, is this management plan written down for you completely or in part or did you receive verbal instructions?
- Have you been told by a doctor whether your asthma is mild, moderate, or severe?
If yes, is it mild, moderate or severe?
- Have you ever discussed "trigger factors" with your local doctor who mainly looks after your asthma?
- Do you have an asthma action plan?
- Do you have a respiratory specialist or general practitioner or practice of GPs that you usually see for your asthma? How many times do you see this specialist or general practitioner a year?
