A four-point Likert-like scale ranging from 0 to 3 was used to assess stress severity for each item, with 0 representing "no stress"; 1, "mild stress"; 2, "moderate stress"; and 3, "severe stress". CI=confidence interval. Bulk-bill=charge only the amount rebated by the government.
