This letter was co-authored by Dr Richard B Murray, Associate Professor and Planning Director, Rural Clinical School, James Cook University, Townsville, QLD. Dr Murray’s details were omitted from the manuscript of the letter submitted to The Medical Journal of Australia, and so were not printed with the letter.
